1. Agreement to Terms

These Terms of Service ("Terms") constitute a legally binding agreement between you and Nova Prime Physical Therapy, LLC ("Nova Prime PT," "we," "our," or "us"), a Virginia limited liability company, regarding your access to and use of our website (novaprimept.com), physical therapy services, and all related services, features, content, and applications (collectively, the "Services").

By accessing or using our Services, you agree to be bound by these Terms. If you disagree with any part of these Terms, you may not access or use our Services. These Terms apply to all visitors, users, patients, and others who access or use our Services.

We may modify these Terms at any time. If we make changes, we will provide notice by updating the "Last Updated" date above. Your continued use of our Services after any modifications indicates your acceptance of the updated Terms.

2. Eligibility and Account Responsibilities

2.1 Age Requirements

You must be at least 18 years old to use our website and schedule services independently. Minors may receive physical therapy services with parental or guardian consent and supervision as required by law.

2.2 Information Accuracy

You agree to provide accurate, current, and complete information during registration, appointment scheduling, and throughout your treatment. You are responsible for maintaining the accuracy of your information and promptly notifying us of any changes.

2.3 Account Security

If you create an account on our patient portal or other online systems, you are responsible for maintaining the confidentiality of your login credentials and for all activities that occur under your account.

3. Description of Services

Nova Prime Physical Therapy provides outpatient physical therapy, rehabilitation, and related healthcare services at our clinic located at 13995 Metrotech Dr, Chantilly, VA 20151. We serve patients throughout Northern Virginia, including Fairfax, Centreville, Herndon, Reston, South Riding, and Oak Hill.

3.1 Services Offered

Our services include, but are not limited to:

• Orthopedic physical therapy
• Sports physical therapy and rehabilitation
• Post-surgical rehabilitation
• Manual therapy
• Dry needling therapy
• Cupping therapy
• Balance and vestibular rehabilitation
• Other physical therapy and rehabilitation services

3.2 Licensure and Qualifications

All physical therapy services are provided by or under the supervision of physical therapists licensed by the Virginia Board of Physical Therapy. Our therapists maintain all required continuing education and certifications.

5. Appointment and Cancellation Policy

5.1 Scheduling

• Appointments may be scheduled by phone, in person, or through our online scheduling system
• Appointment availability is subject to change and is not guaranteed until confirmed
• We will make reasonable efforts to accommodate your scheduling preferences

5.2 Cancellation and No-Show Policy

• 24-Hour Notice Required: We require at least 24 hours' notice for appointment cancellations or rescheduling
• Late Cancellation Fee: Cancellations with less than 24 hours' notice may be subject to a late cancellation fee of up to $50
• No-Show Fee: Failure to appear for a scheduled appointment without notice ("no-show") may result in a fee of up to $75
• Repeated Violations: Patients with multiple late cancellations or no-shows (3 or more within 6 months) may be required to prepay for appointments or may be discharged from care
• Insurance Note: Cancellation and no-show fees are typically not covered by insurance and are the patient's responsibility

5.3 Late Arrivals

If you arrive late for your appointment, we will make every effort to provide your full treatment. However, your session may need to be shortened to avoid delays for other patients. If you arrive more than 15 minutes late, your appointment may need to be rescheduled.

6. Financial Responsibility and Payment Terms

6.1 Payment Obligation

You are financially responsible for all charges incurred for Services rendered, regardless of insurance coverage. Payment is due at the time of service unless other arrangements have been made in advance.

6.2 Insurance

• We participate with many insurance plans but cannot guarantee acceptance of all plans
• Insurance verification is a courtesy; it does not guarantee payment
• You are responsible for understanding your insurance benefits, including deductibles, co-pays, co-insurance, and visit limits
• You are responsible for any services not covered by your insurance
• If your insurance denies a claim, you are responsible for the full amount

6.3 Co-Pays, Deductibles, and Co-Insurance

Co-payments are due at the time of each visit. Deductibles and co-insurance amounts will be billed after insurance processing and are due upon receipt of statement.

6.4 Self-Pay Patients

For patients without insurance or choosing not to use insurance, self-pay rates are available. Payment is due at the time of service. Payment plans may be available for qualifying patients.

6.5 Overdue Accounts

• Accounts not paid within 60 days may be subject to late fees
• Accounts not paid within 90 days may be referred to a collection agency
• You are responsible for all collection costs, including reasonable attorney's fees, if collection action is necessary
• Unpaid balances may result in suspension or termination of Services

6.6 Accepted Payment Methods

We accept cash, personal checks, and major credit/debit cards (Visa, MasterCard, American Express, Discover). A $35 fee will be charged for returned checks.

7. Patient Responsibilities

As a patient of Nova Prime Physical Therapy, you agree to:

• Provide accurate information: Complete health history, current medications, allergies, and changes to your condition
• Participate actively: Follow your prescribed treatment plan, attend scheduled appointments, and complete assigned home exercises
• Communicate openly: Report any changes in your symptoms, concerns about treatment, or difficulty following recommendations
• Respect policies: Follow our cancellation, payment, and other clinic policies
• Maintain respectful conduct: Treat our staff, other patients, and our facility with respect and courtesy
• Ensure safety: Inform us immediately if you feel unsafe, experience unusual symptoms, or have concerns about your treatment

8. Intellectual Property Rights

8.1 Our Content

All content on this website, including but not limited to text, graphics, logos, images, photographs, videos, audio clips, software, and the compilation thereof (collectively, "Content"), is the property of Nova Prime Physical Therapy or its content suppliers and is protected by United States and international copyright, trademark, and other intellectual property laws.

8.2 Limited License

We grant you a limited, non-exclusive, non-transferable, revocable license to access and use our website and Content for personal, non-commercial purposes only. You may not:

13.2 Patient Discharge

We may discharge a patient from care for reasons including, but not limited to:

• Non-compliance with treatment recommendations
• Repeated missed appointments or late cancellations
• Non-payment of financial obligations
• Threatening, abusive, or disruptive behavior
• Achievement of treatment goals

If we discharge you from care, we will provide reasonable notice and assist with referrals to other providers as appropriate.

14. General Provisions

14.1 Entire Agreement

These Terms, together with our Privacy Policy and any other agreements expressly incorporated by reference, constitute the entire agreement between you and Nova Prime Physical Therapy regarding the use of our website and Services.

14.2 Severability

If any provision of these Terms is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, such finding shall not affect the validity of the remaining provisions, which shall continue in full force and effect.

14.3 Waiver

Our failure to enforce any right or provision of these Terms shall not constitute a waiver of such right or provision. Any waiver must be in writing and signed by an authorized representative of Nova Prime Physical Therapy.
